NASA approved identification (a list follows). Foreign Nationals
may present their Foreign Passport as NASA approved identification.

ACCEPTABLE FORM OF DOCUMENTATION TO CERTIFY CITIZENSHIP AND ONE
DOCUMENT TO ESTABLISH IDENTITY. THE LIST BELOW IS NOT ALL INCLUSIVE.
PLEASE BE ADVISED THAT A SOCIAL SECURITY CARD IS NO LONGER ACCEPTED
AS A VALID FORM OF US CITIZENSHIP. THANK YOU.
Documents Acceptable to Establish Identity * Driver's
license or ID card provided it contains a photograph, name, date of birth,
gender, height, eye color and address. * Photo ID card issued by federal,
state or local government agencies or entities providing it contains name,
date of birth, gender, height, eye color and address. * School ID card
with a photograph. * US Military card or draft record. * Native American
tribal document.
Documents Acceptable for Proof of US Born
Citizenship * Birth Certificate - A birth certificate
indicating the individual was BORN IN THE UNITED STATES. Acceptable
certificates must show the birth record was filed shortly after
birth and it must be CERTIFIED with the REGISTRAR'S SIGNATURE.
* Current or expired U.S. Passport * Record of Military Processing
- If Military service, please provide a copy of DD214. * Native
American Tribal Document
Documents Acceptable for Proof of Naturalized
Citizenship * Current or expired U.S. Passport *
Naturalization Certificate - A certificate of Naturalization
is acceptable of the individual claims citizenship by naturalization.
Documents Acceptable for Proof of Resident
Alien status * Resident Alien (green) card * Unexpired
foreign passport with stamp from INS stating temporary authorization
for lawful permanent residency.
Documents Acceptable for Proof of Foreign
National status * Foreign Nationals may present
their unexpired Foreign Passport as NASA approved identification.
If primary evidence of U.S. Citizenship cannot
be obtained, a hospital birth record and a notice from the Registrar
stating that no birth record exists OR a receipt from the
Registrar proving a recent prior purchase of a certified copy of
the original birth certificate will be acceptable as an alternative
form of proof of citizenship.
